65-year-old male patient with nasal cavity recurrence treated with 60 Gy VMAT (AC) around 4 years after previous radiotherapy with 70 Gy. CIRT (DF) yielded significant clinical benefits w.r.t. the brainstem (− 31.5% Dmax), the optic chiasma (− 34.0% Dmax), the ipsilateral eye (− 30.5% Dmean) and the contralateral optic nerve (− 28.8% Dmean). The maximum dose in the ipsilateral optic nerve was not reduced but the mean dose (− 16.2%). The patient developed no local failure during follow-up
