Case 5. A 49-year-old man presented with intracranial hemorrhage related to a right occipital AVM. The lesion was partially treated with embolization. A nidal remnant was further treated with SRS with a marginal dose of 18 Grays. Thirteen years post SRS, the patient presented hemiparesis. a Flair, (b) T2*, (c) T1 and d) T1 contrast enhanced MR images show a large heterogeneous cavernoma-like lesion with extensive edema. Surgery was judged as too risky and the lesion finally stabilized under steroid therapy
